The International Society of Minimally Invasive and Virtual Surgery (ISMIVS, previously known as International Society of Minimally Invasive and Noninvasive Medicine) was founded in July 2013 at the gala of the 1st Yangtze International Summit of Minimally Invasive and Noninvasive Medicine in Chongqing, China. ISMIVS envisions to accelerating the progress in minimally invasive and noninvasive medicine by promoting research, education, communication, and international collaboration.

The Personality in the Medical Education World 医学教育界的大人物
From the 2020 July issue of our ISMIVS Newsletter, we would like to introduce some pioneers in the field of minimally-invasive and noninvasive medicine. They are not only prestigious in their field but also willing to share their experience and ideas with younger doctors, hence making a great contribution to medical education. This issue features Prof. Tan-To Cheung, a prominent surgeon from the University of Hong Kong.

Prof. Tan-To Cheung is the Chief of the Division of Hepatobiliary and Pancreatic Surgery within the Department of Surgery, Queen Mary Hospital, the University of Hong Kong, and President of the Hong Kong Society of Hepatobiliary and Pancreatic Surgery, Vice President (External Affairs) of College of Surgeons on Hong Kong and Council Member of Liver Foundation. He is currently a Clinical Professor at the University of Hong Kong.
张丹图教授现任香港大学玛丽医院外科肝胆胰外科主任、香港肝胆胰外科学会主席、香港外科医学院副院长(外事)和肝脏基金会理事。他也是香港大学的临床教授。
Prof. Cheung is an experienced academic surgeon and his research interest was focused on disease and cancers in the liver, biliary tract and pancreas. He is the chairman of the Hong Kong Pancreatic Cystic Lesion Management Consensus Meeting 2022, the Hong Kong Consensus recommendation on the management of HCC 2018 and 2021, and the Asia Pacific Consensus Meeting for minimally invasive hepatectomy for primary liver cancer (Hong Kong Statement) in 2016.
张教授是一位经验丰富的外科医生,他的研究兴趣着重于肝脏、胆道和胰腺疾病,以及癌症的治疗。他曾担任2022年香港胰腺囊性病变管理共识大会、2018年和2021年香港原发性肝癌管理共识建议和2016年亚太地区原发性肝癌微创肝切除共识会议(香港声明)的主席。
Prof. Cheung is a world-renowned leading expert in the field of laparoscopic liver resection for primary liver cancer. He is the pioneer of laparoscopic liver hepatectomies in Hong Kong with good outcomes. He completed the degree of Doctor of Medicine in 2018. His thesis was the “Application on Minimally Invasive Hepatectomy for liver cancer.” He is very active and visionary in this field internationally. He was the founding member of the International Laparoscopic Liver Surgery Society and was elected as an Advisory Board Member in 2019. Chairman of Radiology Committee of International Liver Transplant Society 2019-2022.
张教授是世界知名的腹腔镜肝切除治疗原发性肝癌专家。他是香港腹腔镜肝切除术的先驱,且治疗效果良好。他在这一领域非常活跃,在国际上也很有远见。他是国际腹腔镜肝脏外科协会的创始成员,并于 2019 年当选为顾问委员会成员。国际肝脏移植学会放射学委员会主席(2019-2022)。
Prof. Cheung was a recipient of the GB Ong Travelling Fellowship Award 2013 and the James IV Travelling Fellowship Award 2017. He has authored more than 310 peer-reviewed articles and other publications and is frequently invited to lecture internationally and to present in plenary sessions. He is listed as top 1% of HKU researchers ranked by Clarivate Analytics worldwide by citations in 2021. He has mentored a large number of trainees, including post-graduate degrees and overseas academia.

On July 8th, the section "The Development of Minimally and Non-invasive Medicine" was successfully held as part of the "5th Yangtze International Summit of Minimally invasive and Non-invasive Medicine initiated, 2023 ‘One Belt and One Road’ Micro-invasive Medicine Innovation Forum".

David Cranston, Past President of the International Society of Minimally Invasive and Virtue Surgery (ISMIVS) and professor at the University of Oxford, gave a special lecture titled "In the steps of William Osler and the Development of Noninvasive Medicine" Throughout the development of medicine, William Osler believed that "diseases that harm the human body require treatment methods that cause less harm to the body". Focusing ultrasound ablation technology is currently the best way to put this belief into practice, and it has been clinically proven to be a safe, effective and minimally invasive solution.
国际微无创医学会(ISMIVS)往届主席、牛津大学教授David Cranston分享“沿着威廉•奥斯勒的足迹看微无创医学的发展”的专题讲座。纵观医学发展过程,威廉•奥斯勒认为的“伤害人体的疾病需要对人体伤害更小的治疗方法”一直是医学追求的目标,而聚焦超声消融技术正是目前最好的践行者,临床上也已经证实这是一种安全、有效的微无创方案。

Chairman of the China-Australia-Asia Pacific Minimally Invasive Surgery Forum and Professor at the University of New South Wales, Felix Wong, Shares a Special Lecture on "Focused ultrasound surgery-potential new direction." He shared the practical applications of focused ultrasound thermal ablation and non-thermal mechanical ablation, as well as the future directions and prospects of HIFU treatment.
中国-澳洲-亚太地区妇科微创论坛主席、新南威尔斯大学教授黄胡信分享“聚焦超声治疗的热消融和非热消融”的专题讲座。他分享了聚焦超声的热消融和非热机械消融的实践以及未来HIFU治疗的方向和未来。

"Former President and Current Director of the European Society for Gynaecological Endoscopy (ESGE), Rudy Leon De Wilde, shared a special lecture on "Forget the Surgical Therapy in Adenomyosis." Focused ultrasound ablation therapy for adenomyosis has undergone evidence-based clinical research and multicenter prospective studies. With numerous clinical experiences, it has rapidly developed, demonstrating good safety and effectiveness comparable to minimally invasive surgery.
欧洲妇科内镜学会(ESGE)前任主席&现任总监Rudy Leon De Wilde分享“子宫腺肌症的治疗:忘掉手术”的专题讲座。针对子宫腺肌病的聚焦超声消融治疗,进行了循证临床研究,也做了多中心前瞻性研究,在诸多临床经验下得到快速发展,已经建立了良好的安全性,并且拥有不低于微创手术的有效性。

Professor Xin Shelley Wang from the Department of Symptom Research at The University of Texas MD Anderson Cancer Center shares a special lecture on the 'Utility of ePROs in Postoperative Care.' In team-based cancer care models, electronic patient-reported outcomes (ePRO) serve as a comprehensive monitoring tool for all healthcare participants, providing management and monitoring of adverse reactions, complications, functional status, and psychological state throughout the entire cancer treatment process. The ePRO system offers additional functionalities, including rapid aggregation of individual symptom items and symptom scales, quantification and standardization of self-reported data, longitudinal patient tracking surveys for long-term.
美国德克萨斯大学MD安德森癌症中心症状研究系教授Xin Shelley Wang分享“电子化患者报告结局在术后护理中的效用”的专题讲座。以团队为基础的癌症护理模式中,电子化患者报告结局(ePRO)为所有护理工作的参与者,提供对于患者在整个癌症治疗过程中的不良反应、并发症、功能状态和心理状态的全程管理化的监测。ePRO系统能提供额外的功能,包括对个人症状条目和症状量表的快速汇总、量化且标准化自诉数据,对患者纵向跟踪调查,以便进行长期趋势分析。

Chairman of the Board of Directors of the Asia-Pacific Association for Gynecologic Endoscopy and Minimally Invasive Therapy (APAGE), Lee Chyilong, shared a special lecture on the ' the necessity of the certification in non-minimal invasive therapy'. High-Intensity Focused Ultrasound (HIFU) was first developed in Chongqing. Guided by ultrasound, the technology uses high-intensity ultrasonic waves to selectively destroy fibroid tissue within the target area without damaging surrounding normal tissues, achieving minimally invasive treatment for uterine fibroids. This advantage of HIFU makes it one of the important trends of the future minimally invasive treatments. Safety certification requires rigorous scrutiny and assessment before comprehensive conclusions can be drawn from experimental trials.
亚太妇产科内视镜暨微创治疗医学会 (APAGE)董事会主席李奇龙分享“微无创治疗技术安全认证的必要性”的专题讲座。HIFU是由重庆最先开展的,通过超声定位后,利用高强度超声波杀灭靶区内的肌瘤组织而不损伤周围正常组织,实现对子宫肌瘤的微无创治疗。HIFU的微无创优势使其成为未来微无创治疗的重要发展趋势之一。安全性认证需要经过反复的严谨的推敲及考核,才能提出试验综合结论。

Director of the Department of Obstetrics and Gynecology at Nanfang Hospital of Southern Medical University, Chen Chunlin, shared a special lecture on the ‘Minimally invasive and noninvasive technology/artificial intelligence- future vision of diagnose and treatment obstetrics and gynaecology disease’. Focused Ultrasound Ablation Surgery (FUAS) can eliminate periodic pain associated with adenomyosis. By ablating the thickened lesions in the myometrium and reducing the lesion area of endometriosis, ectopic endometrial function is removed. Consequently, the uterine environment for conception is improved and fertility is promoted. Additionally, the lecture also explores the integration of minimally invasive surgeries such as intervention and laparoscopy with artificial intelligence.
南方医科大学南方医院妇产科主任陈春林分享“微无创/人工智能-妇产科疾病诊治的未来憧憬”的专题讲座。FUAS治疗子宫腺肌病可消除周期性疼痛,当子宫肌层增厚病变组织被消融,减少子宫内膜异位症的病灶面积,使异位子宫内膜功能丧失,从而改善子宫受孕环境,促进生育。同时也分享了介入、腔镜等微创手术与人工智能融合的应用。
Chairman of Suining Central Hospital Medical Group, Chen Yongjun, shared a special lecture on 'Review and outlook: progress in the field of endoscopy and future of minimally invasive and noninvasive medicine.' The first generation of surgical procedures involved open surgery, followed by the second generation which introduced minimally invasive surgery. The third generation of surgical procedures involves super minimally invasive surgery, or even non-invasive surgery, such as high-intensity focused ultrasound. Analyzing the trends of change in medicine from the perspective of historical evolution, the lecture explores the future prospects of minimally invasive surgery.
遂宁市中心医院医疗集团理事长陈拥军分享“回顾腔镜发展历程,洞察微无创医学未来”的专题讲座。第一代外科手术为开放手术,第二代外科手术为微创手术,第三代外科手术为超级微创手术,甚至是非侵入性手术,如高强度聚焦超声。从历史演变的角度分析微无创手术对医学未来变化的趋势。

Founder and Chairman of the Focused Ultrasound Foundation, Neal F. Kassell, shared a special lecture on "'Curing with sound" Revered as a "game-changer", focused ultrasound therapy is a highly disruptive non-invasive treatment technology. It has transformed the paradigm of disease treatment and holds the potential to become a revolutionary diagnostic tool, much like magnetic resonance imaging (MRI). There is ample scientific evidence indicating the effectiveness of focused ultrasound therapy. Every year, hundreds of peer-reviewed papers are published, showcasing the benefits of this technology, with papers containing clinical trial data appearing in top medical journals such as the New England Journal of Medicine and The Lancet.

Virginia University neurosurgeon Dr. Shayan Moosa shared a lecture on "clinical applications of focused ultrasound in the brain" The lecture introduces the clinical applications of focused ultrasound therapy in treating brain disorders.

Associate Chief Physician/Chief Physician of the Department of Obstetrics and Gynecology at Peking Union Medical College Hospital, Yu Xin, shared a special lecture on "A surgeon’s perspective on focused ultrasound therapy." The lecture emphasizes the importance of selecting treatment methods that maximize benefits and minimize harm. The best choice is the one that both doctors and patients agree upon. There is no single best method; there will always be room for improvement. It is not necessary to pursue or adhere to a specific approach. Every technique has its advantages and disadvantages, and it is important to complement each other's strengths and weaknesses.
北京协和医院妇产科普通妇科副主任/主任医师于昕分享“外科医生的角度看聚焦超声治疗”的专题讲座。治疗方式选择上应该是扬长避短,趋利避害。医生和病人都接受的方式就是最好的选择,没有最好,只有更好,不必追求一种方式,不必固守一种方式,各种技术都有其长短利弊,应该是取长补短,相辅相成。

HIFU Special Issue 高强度聚焦超声特刊


We are delighted to announce that the International Journal of Hyperthermia, the HIFU Special Issue (i.e. Volume 38, 2021), which was partially supported by ISMIVS, was officially released in September. The Special Issue includes 15 articles written and reviewed by world-renowned scholars and pioneers in minimally invasive and noninvasive medicine, elaborating on the status and prospects of the clinical applications of high intensity focused ultrasound on tumours that occurred in specific organs. We'll share the 15 articles included in the upcoming newsletters, stay tuned!

超声引导下高强度聚焦超声消融治疗子宫阔韧带肌瘤的初步研究
The aim is to investigate the feasibility, efficacy and safety of ultrasound-guided high-intensity focused ultrasound (HIFU) ablation for treating the broad ligament uterine fibroid (BLUF).
本研究旨在探讨超声引导下高强度聚焦超声(HIFU)消融治疗子宫阔韧带肌瘤(BLUF)的可行性、有效性和安全性。
A total of 236 patients with symptomatic uterine fibroids were enrolled and treated with JC200 extracorporeal ultrasound-guided HIFU under conscious sedation between January 2017 and December 2018. The patients’ mean age was 38.6 ± 6.3 years.
2017 年 1 月至 2018 年 12 月期间,共有 236 例有症状子宫肌瘤患者入组,并在静脉镇静镇痛下接受 JC200 体外超声引导 HIFU 治疗。患者平均年龄为 38.6 ± 6.3 岁。
Ultrasound-guided HIFU ablation in the patients was technically successful with one session of treatment. The mean longest diameter of BLUFs was 6.2 ± 2.3 cm. The mean NPV ratio of fibroids was 84.08%± 9.4%. There were no severe adverse events or complications associated with the treatment. That shows ultrasound-guided high-intensity focused ultrasound ablation is feasible, effective, and safe for treating broad ligament fibroids.

The impact of laparoscopic gynaecological surgery training on the technicity index of a developing country centre

The author from Brazil published a paper to compare laparoscopic gynaecological surgery training between a developed country’s reference centre (host centre) and a public reference service in a developing country (home centre), and use the technicity index (TI) to compare outcomes and to determine the impact of laparoscopic gynaecological surgery fellowship training on the home centre’s TI.

Taken together, their results demonstrate that access to high-quality laparoscopic training opportunities in highly experienced reference centres can have substantial benefits. It fosters the professional growth of individual surgeons, promotes high standards and best practices, and favours the structuring and development of new surgical care and training centres in the recipients’ communities, especially in developing countries. Such was the case of the experience described herein, which allowed the home centre to increase their TI based on laparoscopic procedures from 6 to 22% over a period of four years.
